A. General Requirement

1. The following are the enrolment requirements you need to submit.

2. PSA Birth Certificate Original copy and 4 long photocopies

3. If married female, PSA Marriage Certificate Original copy and 4 long photocopies

4. Passport size 1.D picture

• White background

• Blouse/polo with collar

• 4 pcs without name tag

• 4 pcs with name tag

5. School Credentials (refer to B. Additional Requirements)

B.  Additional Requirements:

FOR COLLEGE GRADUATE

• College Diploma - original and 4 long photocopies

• Transcript of Records (OTR) - original and 4 long photocopies

• Certificate of Good Moral Character - original and 4 long photocopies

FOR ELEMENTARY GRADUATE / HIGH SCHOOL GRADUATE:

• Elementary / High School Diploma - original and 4 long photocopies

• Form 138 or Form 137 - original and 4 long photocopies

• Certificate of Good Moral Character - original and 4 long photocopies

FOR ALS GRADUATE

• ALS Certificate - original and 4 long photocopies

• Certification of Grade - original and 4 long photocopies

• School Credential (Any of the following)

C. Student Profile:

Put all your requirement in the specified clear book as stated in number 3 here along with the following:

1. 4 pcs passport ID size, with collar, white background

2. 1 PC white long folder

3. 1 PC. LONG CLEAR BOOK (Select cover color according to course)

• BLUE -Domestic Work NC Il and Japanese Language Course

• RED - Caregiving NC ll and Health Care Services NCI

• YELLOW - Food and Beverage Services NC and Housekeeping NC

• GREEN - Massage Therapy NC and Hilot (Wellness Massage) NC H

• WHITE - Beauty Care NC Il, Hairdressing NC Il, Beauty Care Services (Nail) NC 11 

D. Sponsored Scholars

In addition to those required in letters A to C, submit certification document from the sponsoring company or givernment agency.
